Official High School Transcript
Signed Disclaimer for Funds Disbursement
All Applicants are required to submit a typed Letter/Personal statement of no more than (2) two 8.5x11 pages double spaced with one-inch margins. This statement should answer the following Questions:
Why are you seeking this scholarship?
Why are you deserving of the scholarship aid?
What are your college goals and ambitions?
Are there others to whom you give credit for your status as a promising higher education applicant?
